Volunteer

Urban Discovery Academy believes in the power of community. We must come together and find ways to innovate to advance student opportunity and achievement. If you are interested in volunteering email [email protected] to be processed as an official school volunteer and visit our Parent Volunteer Network (PVN) page to learn more about how to become involved.
Volunteer Requirements
Volunteers should review the guidelines and expectations outlined in the Volunteer Handbook. If you have any questions about volunteering, please ask your classroom teacher or inquire in the school office. Requirements Include:
  1. Fingerprint Scanning: Parents should print or obtain a fingerprint form (from the office) specific to UDA. The livescan facility will need the ORI code from the form, specific to UDA’s fingerprint clearinghouse, which is A1270. You can visit ANY convenient livescan facility, so long as you bring the form with our school information listed. If you need financial assistance with the volunteer process, please let us know.

  2. Tuberculosis (TB) Testing: State law requires all individuals who are in contact with school children at schools be tested and found to be free of tuberculosis. A tuberculin test is valid for four years, and the test may be given by your physician or select pharmacies. Many of the CVS Minute Clinics offer them for approximately $40. Your doctor may be willing to complete a Risk Assessment Questionnaire in lieu of the skin test.

  3. Identification: Present a form of current government-issued photo identification (driver’s license, passport, military ID, other government identification).

  4. Volunteer Paperwork: Complete and submit for approval the UDA Volunteer Application.
Field Trip Driver Requirements
To become a field trip driver for UDA class trips, a valid driver’s license, current registration, and vehicle insurance must also be submitted to the UDA office. You will need to ensure students have a proper car or booster seats, if applicable.
Volunteer Expectations
All visitors MUST check-in at the front office and wear a volunteer badge while at the school. Volunteers should review the guidelines and expectations outlined in the Volunteer Handbook. If you have any questions about volunteering, please ask your classroom teacher or inquire in the school office.
